Social Prescribing Link Workers
Did you know we have a friendly team of Social Prescribing Link Workers at McKenzie Group Practice. The team work with patients giving them time and focus on ‘what matters to me’. They take a holistic approach to people’s health and wellbeing. They connect people to community groups, voluntary sector and statutory services for practical and emotional support.
The team also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.
Social prescribing works for a wide range of people, including people:
- with one or more long-term conditions
- who need support with their mental health
- who are lonely or isolated
- who have complex social needs which affect their wellbeing

The SPLW team have recently acquired an allotment in Hartlepool and are hoping to use it as a community allotment so that patients who are experiencing loneliness, mild anxiety and/or depression etc. have a safe space to enjoy outside space, nature and social contact. They are looking for anyone who has experience of planning green spaces to give them some guidance. Please get in touch with the SPLW Facebook page if you think you could help!
COMING SOON! The team are also in talks with Pets4People looking at the therapeutic benefits that pets can provide. This is another fantastic (non-medical) intervention for the team to prescribe.
